I certify that all information I have provided in connection with my application for employment, including any voluntary questionnaires, is true, complete, and correct. All information and documents submitted before or after this application are incorporated herein by reference. I understand that any false, incomplete, or misleading information may result in the rejection of my application or, if employed, termination of my employment at any time.

I authorize the Company, its representatives, employees, or agents to contact and obtain information from all references (personal and professional), prior employers, educational institutions, licensing authorities, and other relevant sources to verify the accuracy of my application, resume, or interview statements. I hereby release and hold harmless the Company and all such parties from any liability arising from the collection or use of this information.

I understand that all applicants may be required to successfully complete a pre-employment drug test and physical examination as a condition of employment. The Company will cover the cost of these requirements; however, if my employment is terminated by either party before completing ninety (90) days of employment, I agree to reimburse the Company for the cost of the drug test, physical examination, training, and any issued personal protective equipment (PPE). I understand that such costs may be deducted from my final paycheck, where permitted by law.

I understand that this application will be considered active for a limited period, not to exceed ninety (90) days, after which I must reapply to be considered for employment.

I understand that this application does not constitute a contract of employment for any specific duration. In the event of employment, I acknowledge that providing false or misleading information during the hiring process may result in termination.

If hired, I understand that I will be required to provide proof of identity and authorization to work in the United States and complete the required Form I-9 in compliance with federal law.
